This policy explains what information Foundations Natural Health collects through this website, how we use it, and the choices you have. It applies to foundationsnh.com. It does not replace any separate privacy notice you receive as a patient of the clinic. Last updated June 2026.
Information we collect
When you use our contact form, we collect your name, phone number, email address, and any message you choose to send. We ask that you keep messages general and not include medical or health details.
When you book online, scheduling is handled by our practice management system, Practice Better. Any information you enter into the scheduler or intake forms is collected and stored by Practice Better under their own security and privacy practices. The same applies to the patient portal and to Fullscript, the dispensary we use for supplement orders.
Like most websites, our hosting provider may automatically log basic technical information such as IP address, browser type, and pages visited. We do not currently run advertising or social media tracking pixels on this site.

How we share it
We do not sell your information. We share it only with service providers that help us operate, such as our scheduling platform, supplement dispensary, website host, and email provider, and only as needed to provide those services or as required by law.
Your health records
Patient health records are maintained within our practice systems and handled in accordance with applicable Minnesota and federal law. Requests related to your health records should be directed to the clinic.
Security
We use reasonable safeguards to protect the information we collect. No method of transmission or storage is completely secure, so we cannot guarantee absolute security. Please do not send sensitive health information through the website contact form.
Your choices

This site links to third-party services such as Practice Better and Fullscript. Their privacy practices are governed by their own policies, not this one.
